Micro-management gets antsy when I'm not at my desk typing something, the less scrutiny I have on me, the more productive I tend to be.
So when I do get out early on Thursday evenings they don't tend to say much because I've managed to cram 100 hours into 8 days.

So long as I'm around during most of the standard and wind my own clock (the one big plus of being a contractor).
Typical full week is 4 9hr days in which I don't take a lunch break, and then, a lovely half day every Friday for an extend weekend. This 41.5 hrs is mitigated by various holidays, or if I need to take time off (Doctor, Dentist, Vacation, Trist, etc), for none of which I am paid.
Within the 9hr period, I may go on coding-binges for hours on end (as do so many of you). When the problem is solved [or the brain starts to loop], I stop, search for a beverage, visit those using apps I developed (to get feedback before something can go awry): pretty much anything to break the concentration (and thus the loop). And, of course, there's visiting CP.
But actual breaks? Every day is unscheduled. Coffee or tea is something to reach for, robotically, whilst working.

Morning.
I have an 1 hour break and 2, 15 Minute tea breaks.
As much as that sounds great, there isn't much to do if you don't have an appointment to pick up your car or go to the bank.
I get very irretated if I have nothing to do, therefore I never take my breaks. Instead, I leave earlier and expect no one to complain as i only leave half an hour earlier.
This saves me some traffic and I keep busy the whole day. I use to complain, that we only had half an hour a day lunch and fought a grusome battle to get this hour and a half, and only to realize that it was actually useless in my case.
Not taking a break, except for making some cofee twice a day and when "nature" calls in exchange for one and a half hour traffic that i miss, I dont think this is such a raw deal.

If you count visits to the toilet and drinking water, then I have five or more breaks.
Everyday I have 30min lunch break and one or two tea breaks (5-10min).

Absolutely!
If one were really dedicated, they'd bring a laptop along to a facilites visitation. The wise employer supplies a power socket next to each porcelin conveninece, lest productivity be limited by battery life.

No - I was commenting on the previous post wherein a journey to the facilites was deemed a break if it took 15 minutes.
Similarly, if one is TRUELY DEDICATED, one keeps several large jars adjacent to their work station. The fanatic may, in fact, perch on one of said jars. (At this point, I've nearly come full circle).

5 mins per hour Input cycle) to grab a power snack, cup of java or refill the water source.
Whenever on the (Output cycle) typical 2-15 mins depending on type;
....#1 depending on distance to the loo
... #2 depends on how thick the mag/book you grab on the way in.
Once per hour when it's drudge work slogging through old code or paperwork. The stretch is needed. On an interesting project, I may exceed 3 hours before realizing it's necessary to step away!

I, of course, count getting coffee at Starbucks, short lunch, afternoon walk-away, as-needed restroom visits, checking external e-mail, visiting Code Project, jumping in and doing a couple of non work-related tasks, reading industry-related e-papers, etc. And I seem to still be pretty productive. . .

I tend to only work as much as required to accomplish the mutual goals of the client and myself. 
However I will perform inhuman acts of blitzkreig-marathon coding if I am paid in amounts sufficient to convince me to do so. Or if offered ritual human sacrifice. 
Though I do, of course, prefer predominately monetary offerings.
