They are called "Windows" in WPF. Why "good example"? You will spend more time reading them than for writing your own. Better learn WPF itself.
Anyway, this is not an example-finding service. If you have some questions on the techniques, we will gladly try to answer.

I'm looking for something that is already design nicely so i can use it and just modify the data requested textblocks

We would have no idea at all what you would think is a "good" example or what it is that your business rules would need in the way of data entry. It's not like every data entry form is exactly the same.
