c# template and excel file

i need to create template such as:
ID* Firstname** LastName*** phoneNumber****
and read data from excel file to get the following output:

ID 1 Firstname Maha LastName Ali phoneNumber 21226768789
ID 2 Firstname Noor LastName ALi phoneNumber 21320903223

how can i do this?
