Implementing a CV form

In my web application, there's a CV.It is a form which is created by myself.Student can fill the CV form and view it as a simple HTML file. I want to provide the option that people can upload different CV formats to the system as PDF files.And when students fill that form given by myself and click on the CV format they want(which is uploaded by different people),it will display according to that format and design.

How should I implement this option?
