This is impossible (or should be impossible). You can't track another phone with only the basic info of it. Tracking in the way you describe should only be possible by police forces and judicial authorities. Even then it should be only in certain circumstances because it would otherwise be a violation of privacy.
However, if the person would agree or you install an app yourself on the phone you want to track, you could use My Tracks. This is an open source project for recording, tracking and sharing location information in real-time. I think that would be the place to get started.
