customized ObjSendMail.Subject line

I am a newbie so any help would be fantastic!!

I am trying to set up a web form that allows a customized subject line in the e-mail that is acquired from input on my web form.
I am currently using http://www.brainjar.com script and would like the following.

Subject Line to include: Customer name, Contact#, Reason for inquiry

All 3 fields Customer Name, Contact# and reason will be input text boxes.

I have been looking for weeks on how to do this with absolutely no luck at all. Thank you in advance for your time and assistance
