How to add a item to desktop right-click menu

Adding a item to desktop right-click menu using shell extension, it does not work. A registry entry was created successfully at HKEY_CLASSES_ROOT\Directory\Background\shellex\ContextMenuHandlers.When right-click desktop, everything on desktop disappear in seconds.
Is there something special when creating desktop menu?
The function STDMETHODIMP Initialize(LPCITEMIDLIST, LPDATAOBJECT, HKEY) does't return to normal.
