Sub table Management ,Department ,City, State ,Country all are dropdown except Address . I gave values for these tables while Creating Database

Address Type (Main Table is 3 radio Button )
when I Connect this DB with MVC and Cretaed the EDMX File and Cretae one Controller For Insert Edit Delete action .. But Its automatically creating Each form For each table.. I want to Create single Form
and if i click the save Button it will save the datas in that particular table.. i donno What is the data type For Dropdown list which has to be give in model..

Now please any one tell me how to create single form for this table with clear explanation.... ...

How to do this Using MVC 4 and I request you to check My Db I normalized the DB it is Correct or not and Give me suggestion ..
