I would say that the main contributor to out less successful projects were because of promises that were made by our project managers or marketeers after the given correct inputs from the product owners and more specifically the disregard of the inputs given by the product owners and the promise to deliver something in half the time. It has nothing to do with agile.
"Program testing can be used to show the presence of bugs, but never to show their absence."
