It happens to me at least 4-5 times a week using latest version of FF as well.

I think FF went and downloaded updates this morning when I started up, but the layout has been ok since then, It just seems to have gone around lunchtime.
It does happen to me occasionally, but that's maybe once every few months.

Looks like you're not downloading the stylesheet.
Ctrl+F5 will force Firefox to re-grab everything.
I'm making some changes which will hopefully work around this occasional issue.

Also, I have noticed some discrepancy on occasion concerning the colouring of the comment headers. Sometimes comments that have high votes (in the case I noticed it, I think it was 4.12) are not coloured in red. Unfortunately, I can't remember which comment it was...

The voting is due to caching, your vote is registered but because after a refresh you land on different server the vote isn't shown.
Usually it takes around a min for all servers to catch up.
I think a post is colored red for 4.5 and above. You can also tinker with the settings (noise tolerance) on top of the forum a bit.
You'll see if you set it to very low that post will actually become grey.

Caching is the cause of votes not always showing up. We are getting close to having our caching revamped to use a better caching service that will remove these issues.
Messages go red when they are over 4.5.

It seems to me that when an author answers a post in his article's forum, what he is doing is answering a technical question about his article. Why isn't his post considered the equivalent of an answer in one of the programming forums, rep-point-wise?

I can't reply to comments when I'm using Safari from my iPhone. I can't hover a mouse pointer over the comment, so a reply button never shows, so I can't click it.

When your level/points go up and you re-vote on something, does that get recalculated for more points or does it keep what your original vote was? I assume that it's the former.

When you revote your old vote is removed and then a new vote is cast using your current status.
