I agree it really depends. If the customer has clearly stated what the functionality is they want, the design and implementation should be left to the coders (they are after all the professionals you hired because you can't/don't want to deal with the details yourself). Obviously if I'm creating a new product and am going to try to sell it I'll try to target some customers, but in the end if I think feature X is important enough I'll add it even if no one has asked me too.

I've on numerous occasions have automated an onerous manual process into a one click DB frontend to much rejoicing by the end users. They didn't even think about their workflow, they were convinced you had to cut and paste into excel, type in your formulas etc etc.
