I think it's mainly because no one has figured out how to convert the "database" into something reasonable.
Decades and decades of veterans' data, for instance, still sitting there in the file system. Even if we went to the front ends all being modern, all they'd be doing would be RPC calls to run M functions on the back end.
Luckily, my job isn't to program stuff. It's just to point to something someone else wrote and say "Here's the problem. You need TWO spaces after an E."
