Form Content to a word document and add some headings and sub heading with the different styles

I want to enter the details in a window form and on button click i want to create a new word document and copy the content to the doc file as a summary and some tables in that document. and some information want to cature for the storage of database.

Please any one can help me on this problem

im having a form like:

Property Address:-----
Location:-----
Land Marks:-----
Land Rates: Min :---- to Max:------
Date of enquiry:-- -- ----
