please help me reaally important for me

there must be at least 60 questions in this survey and 5 answer excellent good etc. and you will cllick your answers by one by for example first question excellent second question good at least you must 48 question answer so save button accept and you will click save button

how to know which question I answered and which answer I clicked and how to count 48 question you clicked

my databbase mysql how can I design my tablesss

You have way too many questions here. You need to start in on the research and then ask something very specific. I am not sure anyone here is going to do your work for you.

We do not do your homework: it is set for a reason. It is there so that you think about what you have been told, and try to understand it. It is also there so that your tutor can identify areas where you are weak, and focus more attention on remedial action.

Try it yourself, you may find it is not as difficult as you think...
