How to create a window in a monitor?

Hi,
Let me know how to create a window in a monitor which is not attached to Desktop by using VC++ 6.0.
The monitor do not have DISPLAY_DEVICE_ATTACHED_TO_DESKTOP flag.
The monitor is independent display.
Please anyone tell me how to do it.

What is "not attached" to Desktop? Where is should be? Why?
