Please make sure the following:
1. Your connection string has values for all parameters (e.g.

Server=yourServerAddress;Database=yourDataBase;User Id=yourUsername;Password=yourPassword;

2. The user name, you are using to connect to the DB has access permission to the database server or your target database.

Note: You can try with

Trusted_Connection=True; or Integrated Security=SSPI;

if you are using local sql server.
