Android SQLite Database still error

I have made aplication that entering 3 data to SQLite database, but still there is error when I click the ADD button that means to insert to database, Please help me to know the error guys :)

please post the code here or leave it out, we are not downloading sources from other locations.

What's the error that you are facing? How are you trying to address the database? is the database accessible?
