so it says your mysql connection do not use a password. try providing null(empty) password from connection parametres. Connection con = DriverManager.getConnection ("jdbc:mysql://localhost:3306/dblabor","root","");

try changing 127.0.0.1 to localhost. or i think you have entered incorrect port number (3006) instead of 3306 which assigned by default when mysql installs.(if you haven't changed it.)

create a new servlet using new servelt wizard. Do not click on finish at first. click next and there you can add servlet info to the web.xml (check the check box) then open web xml file and examine the auto generated code about servlet mapping. 1.check the method in html form is set to POST or GET if you set method to GET you can't use $_POST['Nic']
2.check input field names are correct
