Disconnects means his/her internet connection disconnected, then in that case browser stores the cookie information in the browser. Again when internet connection is up , and user logs in the same site(url). based on cookie information, server validates is the session valid, if yes it shows the session data or the user as logged in (without asking the credential again).
Now comes to your question, the best way to track the session is through using session object.

are you sure both from and to address exists: most of the time smtp server performs checks the from address is of the same domain and it exists. we too faced such issue, and found that if we send the mail from valid from address which exists and from the same domain the mail goes.

Imagining you are using DB Pool to connect to MySQL DB. If the DB pool size is 10, and for every connection taken from the pool is not closed or returned to pool properly, then the available connections in pool will exhaust and it may appear that DB is down.
hence suggest first to restart the Java application and check.
If the issue is not with the java application, then we need to check the MySQL max connection variable size, the more details about the same can be found/seen in below link:
