Dears, It is exactly as you said: for research/training purposes, not primary care... till now it is a virtual home made project with no patients involved at all. The advice of "redesigning" my project is not applicable, as till now the "project" have not yet existed. I used library tm of R and it gave me somewhat acceptable results, but i was thinking of some more accurate. Can you help me ?
R code below, the variable "crude" is the text
library(tm)
corpus <- Corpus(VectorSource(crude))
dtm<-TermDocumentMatrix(corpus)
mat=as.data.frame(as.matrix(dtm))

about 10 reports, even with dummy data, it is for research / training purposes, NOT primary care.

I'd think of some sort of displaying / highlighting controversial phrases like the one you mentioned for the operator to choose from it and store the pattern in a database. What do you think ? @duncan

@ryanb31 here is my problem i have a table of medication interactions containing field "A" which contains a medication name, "B" the other medication interacting with first one and "Grade" which contains severity of interaction. I have 5 blank text boxes textbox1, textbox2,..etc. I fill the 5 text boxes with the prescribed medications and the run the code to check them. the code should put the text in text boxes in the query "where A=? and B=?". there are 2 ways to do this: either to write the queries one by one for example "A=textbox1.text and B=textbox2.text" i did that and it works fine but it needs to write queries for A=1,B=2 then A=1,B=3 then A=1,B=4 then A=1,B=5 then A=2,B=3 and so on. the other way is to loop, in which i am facing my problem.

if two medication interact together, there will be only only row in the grid view which is row number zero. if they don't interact there will be no rows in the grid view.

the "unsuccessful" loops tests only textbox1 vs textbox2 then repeats the label if rows.count >0
the code with no loops works fine but i need to write it many times for each couple of text boxes

thanks a lot. To use a path relative to app root i used the following and worked fine with me
Dim spath As String = Path.Combine(HostingEnvironment.ApplicationPhysicalPath, "drugs.txt")
Dim drugs() As String = File.ReadAllLines(spath, Encoding.Default)
