Create some new nickname contacts - see if they propagate. Even just slightly change the spelling of a few (they could check/cull for valid contact list entries). If the updates show them perhaps you may wish to write apple a stern letter.

They probably only allow fan mail (at a premium). There is no stern letter option.

Well they have obviously (or maybe you just let them) breached the GDPR so you can sue them.

Did you check under Settings | Privacy | Contacts whether Telegram is on the list?
I see Telegram does have access to my Contacts, even though I can't recall expressly giving it. I've turned off Contacts access now, I'd like to see what happens.
Only way to really know would be to uninstall the app, reinstall, expressly forbid Contacts access, and then check in the Settings whether it's got access anyway.

Assuming all privacy etc is working, could it be iOS converting a phone number or email into a "friendly" name for the app? Of course, if the app is allowed to "see" the friendly name, then there is some leakage.
This should be provable by moving a phone number or email to a new contact. If iOS allows phone-local contacts like android, then try it with one of those.

This is what I was going to suggest. Declining app access to your contacts means it can't just hoover them up wholesale and use them in the app. It can't send everyone in your contacts an email or text asking them to get the app, or transfer all your contacts to a contact list in the app. Sounds like that worked correctly.
But iOS does have a "feature" where it will check your contacts for a number and display the contact name instead of the number. So this could be what is happening. Unless the app is doing a screen capture and converting that to text, it still wouldn't have access to that contact information. I'm not sure if this is an API or an integral part of the OS, but it's caused some misunderstandings before IIRC.

GOG[^] are giving away Serious Sam - The First Encounter for a couple of days.
If you haven't played it, it's a first person shooter with a sense of humour - and an enormous number of bad guys. Most of which are crammed onto the screen with you. They aren't bright, but my word they are quick and viscous! Traps, secrets, all the usual - and while it's on rails it never really feels that way, mostly because you have to fight for most of the way anyway. If you get to a quiet moment ... that's when something nasty is going to happen!
