Well...I did have one design where the PSU was controlled by a small PIC. The hardware guy did tell me that I had to be careful, as the wrong combination of PIC outputs could effectively switch 400VDC onto the 5V rail.
So I hooked that to the main application software checksum failure... and made it switch after a number of failures
We did for a while have an Indian distributor who then produced "his own machine" that had a rather high failure rate in the field...
