whenever I read about this I think about Kerbal Space Program.
Why did I loose control over my unmanned probe? Crap forgot to add solar cells.
Successful mission to faraway planetary body, just need to land back on Kerbal... and I forgot the parachute.
Miscalculated fuel need... and now my manned shuttle is in a stable orbit around the sun with no way of getting back.
Something distracts you while doing a burn... and now that rocket is on trajectory to leave the solar system.

This is for real. I was in a little Great Lakes town a few years ago a week after the annual mayfly swarm. They were still cleaning up the mess. Every surface throughout the town was covered in mayfly bodies. Folks used brooms and pressure washers to clean up.
