...my gf just received her MA in Clinical Mental Health Counseling from Goddard College. Now she still 3000 hours of internship to do in NY state become a licensed Mental Health Counselor.
But I'm very proud of her!

I'm pretty sure the licensing authority differentiates between certified time and time with someone who's been certified.

An alternative would be to have the MD's pay for their services out of pocket on an a la carte basis - or do the work themselves.

In the US a Medical Internship (which I'm assuming is what she'll be doing) is indeed a paid position.
The internships of the non-paid variety are usually of the "summer internship for college students" type, and generally last a few months. That's been my experience at least.
