I think he was referring to the Apollo 1 disaster as smart people making a mistake.

That fire happened because 1) systems engineering decided high oxygen was good - less weight and 2) there was an electrical short.
Completely different.
Now, if they were supplying pure oxygen to an airline cockpit, that's full retard.

Yes, and ready to feed any type of fire.
2) there was an electrical short
With probably over 10 miles of wire in the capsule, the likelihood of a short
had to have been considered, then rejected.
Both events were the result of poor choices and never should of happened in my opinion.

At least the Apollo astronauts were not smoking in the cabin. That would also be full retard. According to Wikipedia the increase risk of fire in an oxygen rich environment was previously known w/ several such "mistakes" already having occurred prior. Also according to article providing a nitrogen mixture poses risks re/ bends and something to do w/ partial pressure and hull pressure which I did not quite follow. - Best
