A big problem is the conversion from a written word to a pronounced word (phonetic transcription). Malayalam often writes more consonants/ syllables than are actually spoken. Normally I'd suggest to create a dictionary of written form -> pronounced form. But Malayalam is an agglutinating language, hence you practically cannot do that. You have to find an algorithm for that; and then an algorithm for combining words in a sentence (because often the pronunciation of last/first phonem is changed).
And when you got the phonetic transcription, the next problem is whether the synthesizer copes with all the special sounds of Malayalam, e.g. the zh in "pozham" ("banana")...
