In address there is multiple value like (New work, Boston) and Phone 1 & Phone 2 (Landline & Mobile Number)

Maybe start by asking a proper question; it is far from clear what you are trying to do.

Create a "contact" (or Party) entity for phone1, phone2, etc.; e.g. Home, Business, Next-of-kin / emergency, etc.

To get to 3NF, you'll have to make decisions: how far do you go with "breaking up" the address? Country is simple - only 195 options. County / region? Well, 51 US States, 48 UK Counties, 18 Regions for france (each of which has 13 Departments), and so on ... Cites? Towns? Villages? Loads, and loads, and loads.

To get to 3NF you have to decide what "granularity" of information you consider suitable for "non-duplication" - and invest some time, effort, and / or money in providing information to that granularity. Because if you don't, then you will get duplication when users enter new addresses that are spelled slightly different, or with a hyphen, or a quote, or in the wrong column ...

We can't decide for you - we have no idea of your app or data usage beyond exactly what you typed ...
