If you're using MFC, do this:
LVITEM lvi;
lvi.mask = LVIF_IMAGE;
lvi.iItem = nItemIndex;  // The index of the item to get the image of
m_listCtrl.GetItem(&lvi);
// lvi.iImage will be the index of the image, starting from 0
If you're not using MFC, do it this way:
LVITEM lvi;
lvi.mask = LVIF_IMAGE;
lvi.iItem = nItemIndex;  // The index of the item to get the image of
SendMessage(hwndListCtrl, LVM_GETITEM, 0, (LPARAM)&lvi);
// lvi.iImage will be the index of the image, starting from 0
